Document revision log template and CSV builder
Record what changed, why, when it takes effect and where approval is documented. This browser tool creates a CSV; it does not provide signatures, identity verification or an immutable approval system. Use your organization’s actual document-control requirements.
Download the blank CSV template without JavaScript, or build entries below. Entered text stays in this page and is not saved automatically. Download before leaving.

CSV cells that could start spreadsheet formulas are prefixed with an apostrophe on export. Review imported text and date formatting in your spreadsheet. No approval is implied by an example, a typed name or this download.
What belongs in the record?
Use a stable document ID, an unambiguous revision sequence, the changed section, a useful summary and the reason for the change. Keep the effective date distinct from the drafting date. Link to actual approval evidence rather than inventing a signature.
How this differs from file history
Automatic save history records working states. A controlled revision record explains approved releases and their impact. Neither a CSV nor a version number alone proves conformity with a standard. See the document revision-record guide for a worked example and source-checked discussion of ISO guidance and SharePoint history.
